Team Nomination

Thank you for your interest and participation in this process. Following are the steps for nominating an team for a 2020 award:

  1. Please download and refer to the Nomination Guidelines for submission requirements.
  2. Download the Nomination Letter Template (it is a Word document).
  3. Complete your Nomination Letter at your convenience (note the submission deadline is Friday February 21, 2020).  Please be sure to save it under a new name (not as the template).
  4. Return to this page and complete the submission information below.
  5. Upload your saved Nomination Letter. (PDF format is preferred.)
  6. Submit your nomination.
